We've been having a lot of problems lately with mate constraints flippin from mate to align when adding a concentric contraint to mate diameters.

Also having problems with disapearing contraints in large assemblies.

Can anybody tell me what I doing wrong here?

Karen, there can be lot of reasons for it like how have applied the mates, dependencies of the part, assembly size, complexity of parts, etc.

CADimensions, Inc. is conducting a free SolidWorks Webcast on "Mates and Mating Tools, Best Practices" on Friday 12pm EST. You can register and check if it can be helpful to resolve your issues.
Could there be issues since most files a duplicated by pack and go, and save as copy?
Karen though not sure but yes that can be one of the possibilities. SW might be trying to recognize a file to different file with the same name.
